Practice Brochure

Our practice brochure is a handy summary of the services we offer along with many useful telephone numbers and web addresses. It also gives you the names and qualifications of all the doctors and other medical staff working here, surgery times and similar information.

Registration Form

This is the standard NHS form (GMS1) used to register with a GP. If you would like to join our practice please complete this along with a New Patient Questionnaire and bring them to our Recepion Desk with some photographic ID and proof of address.

New Patient Questionnaire

We ask you to complete a New Patient Questionnaire when you register with us so that we can check any information that we receive from your last GP and make sure that your personal details are correct on the NHS systems. We will also invite you to come to surgery for a health check with one of our Health Care Assistants. This will help us to make sure you get the most appropriate medical care.

Easier Appointments

This small booklet explains the various ways you can make appointments to see our Clinical Team. This includes booking online, using our automated telephone system, calling our Reception Team or even in person at the front desk.

About Your Referral

Sometimes it may be necessary to refer you to a specialist for treatment or advice about a particular medical condition. This booklet explains the referral process and how to use the NHS eReferral online booking system if your specialist makes appointments available on it.

Fees for Non-NHS Services

Whilst you will not be charged for your NHS medical care, there are certain things that are not provided on the NHS and this sheet shows the charges we make in such cases.

Making a Complaint

We make every effort to ensure you are treated fairly, with respect and with dignity at all times. If you should feel that something has not been dealt with well and our staff have not been able to resolve the problem with you, we do have a process whereby you can make a formal complaint. You can be assured that the issue will be fully investigated and that you will receive a written reply, usually within two weeks.

EPS Nomination Form

Use this form to tell us where you would like us to send your prescriptions if you want to use the EPS Service. You can also complete this form and hand it to your Pharmacist.

Application to Access Medical Records Online

Use this form if you want to be able to access your medical records through Patient Access, our online system. You can continue to use Patient Access to book apointments and order medication without needing access to your records. This is a new service availble to patinets aged 16+ from 1st April 2016 and gives you access to parts of your medical history, test results etc.
